Atlanta Healthy Start

The Atlanta Healthy Start is designed to  provide outreach, care coordination, and health education to pregnant and postpartum women by:

  1. Reaching pregnant women and enrolling  them into our case management services

  2. Provide home visitation services to pregnant and postpartum women

  3. Link pregnant and postpartum women and their families to health and social services

  4. Health education through program activities, such as group prenatal care sessions

  5. Leverage the Atlanta Healthy Start consortium to address system fragmentation and any barriers to accessing resources
